Consent record IDs use a fixed format: `<contact point name>#<communication subscription consent type id>`. Any value that you enter in the Consent Record Name field doesn’t take effect.

1. From the Preference Manager dashboard, under Preference Manager, click **Consent Templates**, and then click **New Consent Template**.
2. On the Data 360 tab, select the Subscription template type, and click **Next**.

Forms generated from a subscription consent template support consent for either the Email or SMS engagement channel type.
3. Under Data Stream Source, from the dropdown, select the Salesforce Data 360 source org from where you’re ingesting consent information.
4. In Engagement Channel Type, select **Engagement Channel Type** as the data stream and then click **Update**.
5. In the Engagement Channel Type search field, enter and select the engagement channel type that you want to include, such as email.
6. Click **Update**, and then click **Next**.
7. Under Communication Subscription Channel Type, from the Data Stream dropdown list, select **CommSubscriptionChannelType** and select **CommSubscription** for the display name.
8. Click **Update**. Your Communication Subscription Channel Type options from Data 360 populate.
9. Move the Communication Subscription Channel Type objects that you want to include to the Selected panel, and then click **Next**.
10. Give your consent template a name, and click **Save & Activate**.
11. Navigate to the Privacy Center Preference Manager tab.
12. [Create a template-based preference form](https://help.salesforce.com/s/articleView?id=xcloud.preference_manager_create_form.htm&type=5&language=en_US) using the Data 360 Subscription Consent template.
