# Extend Data Capture with Custom Forms

Enhance your data collection by creating Data Capture Forms and associating them with Timesheets or service resources.

1. Create your Data Capture Forms.
2. From **Setup**, select **Object Manager**.
3. Find and select **Time Sheet** or **Service Resource**.
4. Select **Page Layouts**.
5. Select the existing layout that you want to update.
6. Add the **Form** entity to the related list on the layout.
7. Associate the Data Capture Form with the Time Sheet or Service Resource by using one of the following methods:
    
    1. **Manual**: Open a **Time Sheet** record, go to the **Forms** related list, and manually create a form.
    2. **Automatic**: Create a record-triggered flow for the **Time Sheet** object that automatically associates the data capture form while creating a time sheet.
    
    The Data Capture Form name and description appear to technicians as the last card on both the **Timesheet** and **Home** pages in the mobile app.
