# Create a Recommendation Form

Create a survey for the Admissions: Invite Recommender to Complete Recommendation Survey flow to send to recommenders.

| To create surveys: | Education Cloud Full Access |

1. From the App Launcher, find and select **Surveys**.
2. Click **New**.
3. Enter a name for the survey, and then click **Continue**.
4. Enter a welcome message for your survey.
5. Click **Add Page**.
6. To create questions for your survey, click **Add Question**, and then select the type of question that you want to add. See [Create and Customize Surveys](https://help.salesforce.com/s/articleview?id=xcloud.concept_create_surveys_overview.htm&type=5).
7. Click **Thank You Page**, and enter a thank-you message.
8. Next to Advanced Settings, click ![Actions menu icon](https://sf-zdocs-cdn-prod.zoominsoftware.com/tdta-sfdo-education_cloud-264-0-0-production-enus/89403874-2a89-4126-b574-9db175c14f28/education_cloud/images/actionsmenu.png), and then select **Merge Field Variables**.
9. Expand **Merge Field Access Level**.
10. In Define Merge Field Access, select **System Context - Ignore record-level access**.
11. Expand **Object Variables for Merge Fields**.
12. For Associated Object, enter the details.
    1. Enter Application as the variable name.
    2. Select **Individual Application** as the object.
    3. Save your changes.
13. For Participant Object, enter the details.
    1. Enter Recommender as the variable name.
    2. Select **Contact** as the object.
    3. Save your changes.
14. Click **Back to Builder**.
