# Generate Evaluation Form

Use OmniStudio to package your active questions into a functional evaluation script.

### Required Editions

1. From the Service Console, go to **Assessment Questions** and click **Select Questions**.
2. In the assessment, enter Quality Management and add provide these details:
    
    - Name: A unique name
    - Language: English
    - Type: Enter a type such as Quality
    - Sub Type: Enter type such as Form
    
    For a given combination of Name, Type and Sub Type, only one evaluation form version must be active.
3. Select an assessment question category in the **Create Form** tab and drag the questions into the **OmniScript** tab.
4. Click **Add Steps** to structure the steps into logical sections to guide the analyst and use names for categories and steps.
5. To build the evaluation form, select **Build OmniScript**.
6. Preview the evaluation form.
7. Submit the changes and **Activate** the form.

You can create and manage any number of evaluation forms for different assessment needs.
