# Set Up a Wellness Questionnaire for Health Verifications

You can access and set up a Wellness Questionnaire for Health Verifications.

To access and set up the Safety Cloud Wellness Questionnaire, complete these steps.

1. From the App Launcher, enter Surveys, and then select Surveys.
2. Find the Wellness Questionnaire, and select it.
3. Click **Open Latest Version**.
4. To view a page in the Wellness Questionnaire, from the navigation on the left, select the page.
5. To create an editable version of the questionnaire, from the dropdown menu in the upper right corner of the user interface, select **Create New Version**.
