# Conduct Change Risk Assessment for IT Services

Conduct a risk assessment to understand the potential impact of a change before you implement it. By evaluating factors such as urgency, scope, and affected systems, your team can make informed decisions to minimize disruptions and protect service quality.

### Required Editions

1. From the Change Request Record page, go to the **Risk Evaluation** tab.
2. Click **Start** to start the evaluation.
3. On the Risk Evaluation Questionnaire page, answer the questions and click **Next**.
4. Answer the questions on next page and click **Next**.
    
    You can review the calculated risk level and risk score on the Risk Score card.
5. To retake the questionnaire, click **Retake** in the **Risk Evaluation** tab.
