Deploy this template to provide employees with a standardized way to update existing performance goals.

This template creates a service request record that captures essential user details for accurate and auditable fulfillment. Review what’s included with the template.

## Intake Attributes

The intake form for this template captures these details from the employee: Goal ID, Subject, Case Origin, Title, Due Date, Started With Employee IDs, Description, Completion Date, Percent Complete.

## Fulfillment and Integration

This template includes preconfigured integration through the BambooHR connector for intake and/or fulfillment tracking. Custom logic, routing, or approvals can be managed using Flow Builder.
