Set Up Intelligent Care Management with a Conversational Assistant

Set up Intelligent Care Management through a simple conversation with your AI assistant instead of navigating multiple setup pages. Validate prerequisites, assign recommended permissions, enable the Discovery Framework, and configure Care Plans, Goal Assignment, automatic sharing, and Care Gap Detection. Optionally, enable AI-powered assessment features and MCG Care Guidelines. Add assessment, care plan, and care gap components to the Patient Account page to help care coordinators manage patient care. Validation at each stage helps you complete setup more quickly and with fewer configuration errors. If you skip an area, you can return later and resume setup based on your org’s metadata.

Where: This change applies to Lightning Experience in Enterprise and Unlimited editions with the Health Cloud and Agentforce for Health Cloud add-on licenses. You can run the setup skills using an AI coding assistant, such as Agentforce Vibes or Claude Code, that's connected to the Headless 360 MCP Server (Beta).

Headless 360 MCP Server is a beta service that is subject to the Beta Services Terms at Agreements - Salesforce.com or a written Unified Pilot Agreement if executed by Customer, and applicable terms in the Product Terms Directory. Use of this beta service is at the Customer's sole discretion.

How: Use the Headless 360 MCP Server (Beta) to connect your AI coding assistant to your org. Then enter a plain-language prompt, such as “Set up Intelligent Care Management” or “Configure ICM.” The assistant identifies the appropriate skills and walks you through the configuration. For MCG Care Guidelines integration, securely configure the named credentials, API endpoints, decision tables, and integration definitions. To enable Care Gap Detection, define clinical measures that align with your organization’s protocols before care gaps are shown to users.
