Use the Health Cloud External Care Gap Integration data kit to ingest and map care gap record from external Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems to standard Health Cloud data model objects. Populate the pre-packaged Ingestion API streams in your org and unlock the schemas and data mappings required for patient and member outreach. Trigger existing segments, data graphs, and automated email or SMS flows for all patients, regardless of whether the care gap records reside in Salesforce or an external health system.
Where: This change applies to Lightning Experience in Enterprise and Unlimited editions with the Health Cloud and Data 360 add-on licenses.
When: This feature is available to users with the Health Cloud Foundation, Data Cloud Architect, Data Cloud User, Marketing Cloud Manager, and Member Self Service Agent Access permission sets.
How: From Setup, go to the Health Engagement Settings page, and turn on Patient and Member Outreach. Then, follow the guided steps in the Set Up Data 360 for Health Engagement Patient and Member Outreach section.
