# Preventive Care Agreement Fields

Use these fields to define payment terms for a contract of agreement type Preventive Care Agreement. This agreement type supports the Preventive Care payment type in which providers are paid a bonus amount when they reach an agreed upon target. Targets can include achievements such as reducing admission or readmission rates to a certain percentage.

| Field | Description |
| --- | --- |
| Contract Payment Agreement | The related contract payment agreement record. |
| Bonus Measure Type | The measure type used to define the bonus payout. For example, Preventive Readmission Rate or Preventive Admission Rate. |
| Bonus Measure Description | The description of the bonus measure type. |
| Current Measure Percent | The current percentage of the specified measure. |
| Target Measure Percent | The expected percentage of the specified measure. |
| Bonus Payout | The bonus paid on reaching the target measure percentage. |
| Additional Information | Additional information on the bonus payout and target expectation. |
