# Capitation Care Fee Agreement Fields

Use these fields to define payment terms for a contract of agreement type Capitation Care Fee Agreement. This agreement type supports the Capitated Payment type in which providers are paid a fixed amount for each member every month for a fixed set of services. As part of the program, the payer can also decide to withhold some amount and pay it later as a bonus amount based on the performance.

| Field | Description |
| --- | --- |
| Contract Payment Agreement | The related contract payment agreement record. |
| Monthly Fee | The fee, agreed upon by the provider and payer, for each member per month. |
| Monthly Withheld Percentage | The percentage of the monthly fee that's withheld by the payer and shared later with the provider based on performance. |
| Monthly Balance Fee | The balance fee for each member after the monthly withheld fee. |
| Member Age Range Lower Limit | The lower age limit of the enrolled member for the capitation care fee agreement. |
| Member Age Range Upper Limit | The upper age limit of the enrolled member for the capitation care fee agreement. |
