# Care Plan Templates Help Simplify Onboarding

Take the “work” out of workflows with care plan templates that let care coordinators create customized care plans with just a few clicks. Coordinators can add problems, goals, and tasks in bulk to a care plan. Predefined care plan templates reduce the time spent to create a care plan and enforce standards of care that improve healthcare outcomes.

    You can use care plan templates to create standardized and reusable care plans, and add template problems, goals, and tasks for specific health needs.

    Care plan template problems are health diagnoses, such as high blood pressure or diabetes, that can be reused for different patients. Care coordinators can use care plan template problems to provide standardized care across similar problems and patients. You can create a care plan template problem to use in your care plan template.

    Care plan template goals are common health targets that care coordinators can reuse in different care plans. Care coordinators can add predefined goals to a patient’s care plan, saving time and effort. For example, a care plan template goal for patients with type 2 diabetes is Dietary Changes, with details of low-sugar meal plans. You can create a care plan template goal to use in your care plan template or template problem.

    Tasks are actionable steps a patient can take to achieve a goal and overcome a problem. For a type 2 diabetes patient, a task could be testing blood sugar levels weekly. Each task is assigned to a care team member who makes sure the patient completes the care plan tasks. With a care plan template task, you can easily add predefined tasks for a specific problem to a patient’s care plan. You can create a care plan template task to use in your care plan template, template problem, or template goal.

    Use Data Loader to import existing care plan templates into Salesforce and make them available to care coordinators. Data Loader is a client application that reads, extracts, and loads data from comma-separated values (CSV) files or from a database connection.
