# Create Provider Cards for a Facility

Design cards to show your users information about providers and the facilities where they work. For example, a network specialist may need to know at a glance which facilities Dr. Carol Peterson is practicing at and when she is there.

This card type will live on the Contact record page for practitioners. We’ve set up the information sources to support this information:

- Effective To
- Effective From
- Operating Hours
- Time zone
- Payer Network Name
- Network Type
- Driving Directions

1. In Setup, go to Relationship Card Type Manager and, in the dropdown next to a card that you've created (such as Doctor-Hospital Relationship), click **Edit Card Fields**.
2. For **Indirect Source Object**, select Healthcare Practitioner Facility, then select the **Effective To** and **Effective From** fields and click **Add Selected Fields to Card**.
3. Now we'll go get the operating hours information from the Operating Hours object, which is directly related to our base Healthcare Practitioner Facility object.
    1. Click **Select fields from parent or child object related to HealthCarePractitionerFacility**.

These are the fields that enable the relationship from the initial object.
    2. Under **Select Object**, choose Operating Hours, and under **Select Field**, choose the OperatingHoursId lookup field.
    3. Select **Name** from the list of fields, and click **Add Selected Fields to Card**.
4. Get the insurance plan information from the Healthcare Payer Network object, which is related to our base Healthcare Practitioner Facility object through the Healthcare Facility Network object.
    1. Click **Select fields from related object two levels away from HealthCarePractitionerFacility**.
    2. Under **Select Object**, choose Healthcare Facility Network, and under **Select Field**, choose the FacilityNetworkId lookup field.
    3. In the next **Select Object** field, choose Healthcare Payer Network, and under **Select Field**, choose the PayerNetworkId lookup field.
    4. Select **Network Type** and **Payer Network Name** from the list of fields, then click **Add Selected Fields to Card**.
5. Get the driving directions from the Location object, which is related to the Healthcare Practitioner Facility object through two intermediary objects: Account and Associated Location.

    1. Click **Select fields from related object three levels away from HealthCarePractitionerFacility**.
    2. Under **Select Object**, choose Account, and under **Select Field**, choose the AccountId lookup field.
    3. In the next **Select Object** field, choose AssociatedLocation, and under **Select Field**, choose the ParentRecordId lookup field.
    4. In the third **Select Object** field, choose Location, and under **Select Field**, choose the LocationId lookup field.
    5. Select **Driving Directions** from the list of fields, then click **Add Selected Fields to Card**.
    6. Save your work.

The fields you've selected are shown in the panel on the right. Change their order and rename them as needed. For example, "Name" isn't a very intuitive label for the field that displays the doctor's available hours. You might want to rename that to something like "Hours Available."
6. In Setup, go to the Lightning Application Builder and select Contact.
7. Drop the Relationship Card component from the left panel onto the Account record page.
8. In the Configuration panel on the right, specify a heading for the tab.

The default heading is “Relationship,” but you may want something more specific, such as “Health Care Providers.”
9. For **Card Type**, select the name of the card you created that you want to show on this page.
10. Save the page.
