Create code sets, units of measure, and care metric targets to set up remote monitoring. Enable care coordinators to remotely monitor health metrics by displaying care observations and remote monitoring charts on the patient’s account page.

    Code sets define the metrics that providers monitor in patients, for example, blood sugar, cholesterol level, and amount of exercise. Define the code sets your providers use with their patients.
