Note The delivered fields in the Patient Card component, with the exception of Campaigns, show data from the EHR Data Model. When you upgrade to the Clinical Data Model, we recommend deleting the EHR Data Model records, then creating records based on Clinical Data Model objects.

1. Create a formula field to filter which records to display. You enter the name of this field in the Override Filter Field when you create a Patient Card Configuration record.

    1. Create a checkbox formula field on the object you want to pull data from, for example, Health Condition.
    2. Enter a formula that returns true or false. For example, to display records from the Health Condition object where the status is Active or Relapse, enter the formula `IF( OR(( ISPICKVAL( ConditionStatus , "Active") ),( ISPICKVAL( ConditionStatus , "Relapse") )) , true, false)`.
    3. Save your work.

Note See [Sample Override Filter Field Formulas for the Clinical Data Model](https://help.salesforce.com/s/articleView?id=ind.admin_patient_card_cdm_templates.htm&release=264.0.0&type=5 "Use these formulas as is, or as a starting point to create your own formulas, to filter records in the Clinical Data Model.") for formula examples.
2. Create a Patient Card Configuration record, and enter the name of your formula field in the Override Filter Field.
    1. From the App Launcher, find and select **Patient Card Configurations**.
    2. Click **New**.
    3. Enter values for each field:

Considerations when adding fields:

- Consider specifying a field in Field Name that’s required in the target object. Even if a record meets the criteria to appear on the Patient Card, it doesn’t appear if the value of the field you specify for Field Name is null.
- The text you enter in Friendly Field Name isn’t available for localization using the Translation Workbench. For this text to appear in another language, clone the configuration record and enter a new Friendly Field Name value using the language that you want to display. Then set the Language field of the new record to that language. The system displays the label text that matches the user’s language setting.
- If you clone a configuration record to localize the label, modify the sort order for the new record. You can’t have two records with the same assigned sort order, so create a different version number for the new record. For instance, if the English record has 4 in the Sort Order field, then assign the Spanish version 4.1.
- When you create two Patient Card Configuration records from the same object, the fields are displayed next to each other regardless of the sort order.
